Why we need this information.
We need to collect additional information so we can move quickly when your requested by a site!
- We need your legal first and last names and birthdate to purchase your plane tickets.
- Even though we’ve discussed your availability start day, we’d like to confirm.
- We ask for your professional license information, because we’d like to check and ensure everyone has the appropriate certifications before committing to a site.
- Up-to-date CPR cards are required at the sites. We’d like a copy of those as well!
- We’re asking for your drivers license to double check your spelling of your name…and we’ll use it later on as a form of identification on your I-9 documentation.
